POLICY AND LIABILITY RELEASE WAIVER 

TERMS AND CONDITIONS

I understand and agree that even with social distancing and sanitization protocols, there is a possibility of contracting the SARS-Cov-2 (Severe Acute Respiratory Syndrome coronavirus 2) virus which can lead to the COVID-19 (coronavirus) disease while attending programs at Music Together YEG/ Music Together Edmonton Online.Therefore, I voluntarily agree to assume all risks and responsibility of contracting the SARS-Cov-2 virus, which I or any of my participating children may contract during any Music Together YEG/ Music Together Edmonton Online classes or activities.

I also exempt and release Music Together YEG / Music Together Edmonton Online/ Music Together, LLC and their owners, employees, sub-contractors, instructors, assistants, volunteers, location vendors, and/or program participants from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury or death to me, my children or property which may arise out of or in connection with SARS-Cov-2 and COVID-19.

I understand that Music Together YEG/ Music Together Edmonton Online is not requiring proof of full vaccination from participants. PROPER MASKING (full coverage of nose, mouth and chin) is not mandatory but IS HIGHLY RECOMMENDED for everyone ages 3 years and up at all times during every class.

I agree to adhere to class community sanitization protocols which will include mandatory hand sanitization at the beginning, during and at the end of each class.

I agree and give full consent to active symptom screenings for myself and my participating children which may include a temperature check and understand that a temperature reading of less than 38 degrees celcius is required in order to attend class

On the day of each Music Together YEG/ Music Together Edmonton Online in-person class that myself and my children are to attend, before arriving to the location of my Music Together YEG in-person class, I agree to perform a symptom check for myself and all participating children as per the provided Daily Health Screening Checklist and agree to answer the checklist truthfully and honestly to the best of my knowledge. 

Should I answer “YES” to any of the questions on the provided Daily Health Screening Checklist for either myself or any of the participating children, I agree to NOT attend class that day.

I agree to alert Music Together YEG/ Music Together Edmonton Online if I or any of the participating children have tested postivie for COVID-19 or have been in close contact with anyone that has tested postive for COVID-19 within a 2-week timeframe of having last attended one of our in-person classes

I agree to alert Music Together YEG/ Music Together Edmonton Online of any and all medical conditions and allergies that I or any of the participating children listed above have. I understand that it is especially important to alert Music Together YEG/ Music Together Edmonton Online of any health conditions which may cause similar symptoms of SARS-Cov-2 and/or COVID-19. These health conditions and symptoms include but are not limited to a cough and/or shortness of breath due to asthma, and/or sore throat, runny nose and/or nasal congestion due to severe seasonal allergies

I agree and understand that iI will not receive a credit or refund for missed classes due to illness or adherence to the Daily Health Screening Checklist Stay-at-Home requirement. I understand that in lieu of a credit or refund for missed classes I have access to unlimited make-up classes throughout the semester

I agree and understand that if at any time gathering in groups poses a collective health risk for their clients and staff, Music Together YEG/ Music Together Edmonton Online reserves the right to strengthen safety protocols which may include, but is not limited to, mandatory masking, delaying classes or moving in-person classes online.

I agree and understand that no refunds will be granted should Music Together YEG/ Music Together Edmonton Online exercise their right to take measures to strengthen class safety protocols which may include, but is not limited to, mandatory masking, delaying classes or moving in-person classes online in order to protect their clients and staff from a collective health risk.

I agree to the following terms of Music Together YEG/ Music Together Edmonton Online Cancellation Policy:
More than 3 weeks prior to the first scheduled class of the semeser - FULL REFUND minus a $25.00/child cancellation fee
3 weeks prior to the first scheduled class of the semeser - FULL REFUND minus a $35.00/registered child cancellation fee
2 weeks prior to the first scheduled class of the semeser - FULL REFUND minus a $50.00/registered child cancellation fee
1 week prior to the first scheduled class of the semeser - FULL REFUND minus a $75.00/registered child cancellation fee
Within 1 week after the first scheduled class of the semeser - FULL REFUND minus a $100.00/registered child cancellation fee
There will be no refunds given once the second scheduled week of the semester begins

I understand that class locations, times and instructors are subject to change. I am aware that all changes will be reasonable and will offer an equitable experience and therefore any requested refunds will be subject to the same terms of the Cancellation Policy stated above.

ACKNOWLEDGEMENT OF CONSENT

I am accepting these Terms and Conditions on my own behalf AND for the participating children. I certify that I am the parent or legal guardian and have the right to waive these rights.
